Please complete the following form to request lab services, equipment, materials, and to reserve lab space.

Note: NFSTC lab staff will not initiate any project task until an authorized Laboratory Request form has been submitted. Please notify the Forensic Services Manager in writing of any project plan changes that affect the laboratory.

Note: Lab protocols/procedures may be attached if desired, but you must provide all relevant information using this form. Unless special provisions are made with the Laboratory Manager, the lab staff is not responsible for reviewing protocols to determine the project’s needs.
